Workshop – October 12 and 13, 2012
Day One: Friday, October 12, 2011 - 4:30 pm to 9:00 pm
Activities include:
Small group work, discussions, and review of patient encounters related to Anemia and Thyroid Disorders
Day Two: Saturday, October 13, 2011 - 9:00 am to 4:30 pm
Activities include:
Small group work, discussions, and review of patient encounters related to Diabetes and Hypertension/Dyslipidemia
Support for assignments and technology
You will be guided through the goal setting process and see how one pharmacist has set and integrated practice goals.
Assignments
This four-part assignment has been designed to support you in developing and implementing goals for practice change.
Part A - Assessing Your Practice
The purpose of this activity is to review your current practice to identify what you would like to learn in this course.
Due Date: September 21, 2012
Part B – Setting Your Practice Change Goals
With the support of a facilitator, you will develop personal practice goals that will allow you to integrate what you have learned into your daily practice routine.
Due Date: October 14-21, 2012
Part C - Goal Integration Assignment
Once you have set your personal practice goals, you will demonstrate the integration of those goals into your own practice with a written assignment.
You will not only have the opportunity to share your goal integration process with your colleagues, but also to to receive feedback from your peers and hear what your colleagues are doing in their practices.
Due Date: November 16, 2012
Part D – Post Course Practice Assessment
The purpose of this activity is to re-assess your current practice and re-visit your practice goals after participation in and completion of all the course components.
